Abstract

Penatalaksanaan Hematotoraks Sedang Et Causa Trauma Tumpul Diana Mayasari, Anisa Ika Pratiwi Fakultas Kedokteran, Universitas Lampung Abstrak Hematotoraks adalah adanya darah dalam rongga pleura dan dapat disebabkan karena trauma tumpul atau tajam, juga mungkin merupakan komplikasi dari beberapa penyakit. Hematotoraks dapat bersifat simptomatik namun dapat juga asimptomatik. Tujuan utama tatalaksana dari hematotoraks adalah untuk menstabilkan hemodinamik pasien, menghentikan perdarahan dan mengeluarkan darah serta udara dari rongga pleura. Seorang pasien remaja laki-laki berusia 17 tahun mengalami trauma tumpul dibagian dada sebelah kiri yang dialaminya dalam sebuah kecelakaan. Pada pemeriksaan fisik didapatkan kesadaran Somnolen, GCS 9 (E3V3M3), tanda vital tekanan darah 130/60 mmHg, nadi 100x/menit, kuat reguler, respirasi 28x/menit, pernafasan dangkal, suhu 35,8 0C, SPO2 82%, pupil isokor (3mm), reflek pupil langsung dan tidak langsung mata kanan dan kiri (+), pada leher terdapat jejas bagian kiri, pada dinding dada didapati hasil Inspeksi pergerakan dada asimetris ( gerakan dada kiri tertinggal), retraksi dinding dada bagian bawah kanan dan kiri (+), pada regio anterior toraks sinistra diatas processus xypoideus terdapat jejas ukuran ± 1x5 cm, ictus cordis tidak terlihat dan tidak teraba, perkusi redup pada thoraks sinistra, suara nafas kiri menjauh, vesikular (-/+), ronki basah (+/-) dan pada pemeriksaan rontgen thoraks AP didapati gambaran semi opaq dan sudut costo frenikus tumpul pada thoraks sinistra yang mengarah kepada hematotorak. Langkah pertama yang di lakukan pada pasien ini adalah stabilisasi hemodinamik dengan melakukan resusitasi lalu di lakukan tindakan pemasangan Water Sealed Drainage untuk mengeluarkan darah yang terjebak pada rongga pleura pasien dengan total perdarahan yang didapat sebanyak 750 cc. Kata kunci: hematotoraks, trauma tumpul thoraks, water sealed drainage Management of Moderate Hematotoraks Et Causa Blunt Trauma Abstract Haematothorax occurs when there is blood in the pleural space and can be caused by blunt or sharp trauma, it may also be a complication of some diseases. Hematotoraks may be symptomatic but may also be asymptomatic. The main purpose of treatment of hematotoraks is to stabilize the patient's hemodynamics, stop the bleeding and remove the blood and air from the pleural space. A 17-year-old male patient had a hematotoraks due to the blunt trauma he experienced in an accident. From the physical examination was obtained Somnolen Consciousness, GCS 9 (E3V3M3), blood pressure 130/60 mmHg, pulse 100x/min, regular strength, 28x/min respiration, shallow breathing, 35.8 0C temperature, 82% SPO2. Isokor pupils (3 mm), direct and indirect pupil reflexes right and left eyes (+), on the left neck there is injury on the chest wall found as Inspection of asymmetrical chest movement (left chest movement left behind), retraction of the lower chest wall right and left (+), In the anterior region of the left thorax above the xypoideus processus there is a injury, size of ± 1x5 cm. Ictus cordis not visible and not palpable, dim percussion on the left thorax, the sound of the left breath away, vesicular (-/ +) damp ronki (+/-), and the X-ray examination found a picture of thoracic AP semi opaq and costo phrenic obtuse angle that leads to hematotorak. The first step in this patient is hemodynamic stabilization by did resuscitation and then do the installation action of water sealed drainage to remove the blood trapped in the patient's pleural cavity with total bleeding obtained as much as 750 cc.
